Request an occupational therapy assessment

What happens during an assessment?

We will arrange a time to speak with you and listen to what you need support with. This could be in person at your home or over the phone.

We will ask some questions about your day-to-day life, your current situation and what support you already have in place to see how we can help. We want to find out what’s important to you and how you complete daily tasks. We then suggest how you can make the most of your abilities. We can offer advice on different ways of doing something and signpost you to other teams that will be able to support you.

A family member or friend may join you for the assessment if you would like.

What happens after an assessment?

We’ll provide you with a copy of your assessment and let you know what we can do to help. We may be able to offer support in terms of equipment or changes to your home, or we can identify an organisation who may be more suitable. Any major changes to your home may require a Disabled Facilities Grant. For more information about this, please see ‘Fact Sheet 18: What is a Disabled Facilities Grant?’ on our Factsheets page. If changes are not possible, we can talk to you about moving home to a property that works better for you.
