Request a social care assessment

An assessment will help us to find out what support you need.


What happens during an assessment?

We will arrange a time to speak with you and listen to what you need support with. This could be in person at your home, in hospital or over the phone.

We will ask some questions about your day-to-day life, your current situation and what support you already have in place to see how we can help. You will also be asked about your finances as you may need to pay towards any care we offer.

A family member or friend may join you for the assessment if you would like.

What happens after an assessment?

We’ll provide you with a copy of your assessment and let you know what we can do to help. We may be able to offer support in terms of equipment or a care service, or we can identify an organisation or agency who may be more suitable.
