An assessment will help us to find out what support you need.

What happens during an assessment?

We will arrange a time to speak with you and listen to what you need support with. We will ask some questions about your day-to-day life to see how we can help. We will ask about your current situation and what support you already have in place. You will also be asked about your finances as you may need to pay towards any care we offer.

You can have a friend or family member there with you.  Or you can get someone to help you . We will also talk to other people who know or support you, including your doctor, nurse, family or friends.

What happens after an assessment?

We’ll provide you with a copy of your assessment and let you know what we can do to help. We may be able to offer support in terms of equipment, a care service, or we can identify an organisation/agency who may be more suitable.

You will be asked about your finances during your assessment, so we will also let you know if you need to pay towards your care.
