Planning for a hospital stay as a carer
If you are going into or leaving hospital you should prepare for your visit and know what is going to happen when you leave.
Preparing to go into hospital
Make sure you have:
- told us about the hospital stay
- spoken to your friends and family about what is happening
Request or update your carer's assessment
If you have not already done so, you should request a carer's assessment.

Make sure you have an emergency plan
To make sure that the person you care for is looked after if you're unable to, you should create a contingency plan.
The plan helps to make sure the person you look after will be cared for if you are unavailable because of an emergency.
What happens to the person that you care for
During your stay, you may need someone to help with the person that you care for. During your assessment, we can discuss things that can be put in place.
For example, we can help to organise:
- a paid carer to carry out your role during your absence
- a short stay in residential care
- equipment or telecare
Leaving hospital
When leaving hospital, you may need help to continue with your caring role for a while.
We will help you create a plan so that when you leave, you will know:
- how you will get back home
- the support you might need once you are at home
- the medicine you need to take and how to take it
We will work closely with our NHS colleagues to find out what outcome you want, and help you achieve that outcome.
