Assessments
To figure out what care and support services you might need, we will talk to you about your needs and complete a care needs assessment.
At Buckinghamshire Council, we call this a “Better Lives Assessment.”
The Better Lives Assessment Conversation
You can either refer yourself or call the Adult Social Care team to ask for an assessment. Under the Care Act (2014), we must assess you and provide information, advice, and support if your needs qualify.
- After an initial conversation, one of our social care workers will arrange a visit at a time which is best for you. During the visit we will talk about the things you do at home and outside.
- The visit can take as long as you need to tell us about yourself.
- You can have a friend or family member there with you, or an advocate who can help you understand what is happening.
- Following the assessment, the social care worker will look at your situation based on the eligibility criteria from the Care Act 2014. Then they will decide what kind of help you qualify for.
If you are eligible, we will create a care plan with you that outlines the support you will receive and how it will be provided.
If you do not qualify, we will still offer information and advice.
You can request another assessment at any point.
We will regularly look at the care you get from us. This is to make sure we're giving you the best help we can.
You can tell us how things are going. We call this a "review".
The first review is usually within 6 weeks of your care starting. After this we will review your care every year. You can ask for a review at any time, or if something changes.
The review meeting may be in person or over the phone.
Eligibility
To qualify for care and support, as outlined by The Care Act (2014), your needs must:
- Have a physical or mental impairment affecting daily tasks
- Your condition has a significant impact on your wellbeing and independence
- Your needs cannot be met by existing support networks or community resources. Which means that you cannot do at least two of the below eligibility outcomes.
- eat and drink well
- keep yourself clean
- use the toilet
- get dressed
- be safe at home
- keep your home clean
- to make and maintain relationships
- do work, training, education or volunteering
- use local community services
- look after any children you have
We will arrange the necessary support for you if you meet the eligibility criteria. You will need a financial assessment to determine how much you may have to pay towards the cost of your care.
